Some tips to remember
- If your child has a fever, they need to be fever free for 24 hours without the help of Motrin or Tylenol before returning to school. This helps to stop the spread of infection.
- If your child is vomiting or has diarrhea, they must remain home from school until it stops and they can tolerate food and liquid.
- If your child is absent from school for three (3) or more consecutive days, they may not return to school without a note from the doctor for the dates they missed.
Medical paperwork - Each child in the school must have on file in the nurse’s office; a doctor’s physical which allows them to participate in all activities, a current immunization record for their age, and a health history form completed by the parent. This is a New Jersey State law. Any child who is not up to date with their medical paperwork will be excluded from school.
- The nurse has information regarding free immunizations and physicals for children, please call the school and ask for the nurse if you have any questions.
Children with asthma - If your child has a diagnosis of asthma, they are required to have an asthma action plan completed by their physician and kept on file in the nurse’s office. They are also required to have a medic alert form completed by the parent. These forms are found under Asthma Forms.
- The nurse has a nebulizer machine, but will require tubing and a mask to administer the treatments. It is best if there are two sets of tubing, one for school and one for home.
- All medication is stored in a locked cabinet in the nurse’s office. Medication may not be brought to school by students. It must be delivered by an adult, and labeled with a pharmacy label.
Children with allergies - If your child has allergies to foods or insects, please make sure the nurse is aware.
- Each child with food allergies must have a Food allergy action plan completed by the doctor and a medic alert completed by the parent. These forms are located under Allergies.
- Epi-pens are to be delivered by the parent to the nurse. Please make sure the expiration date on the package covers the entire school year.
Children with any other medical need - Please make sure the nurse is aware of any medical need for your child.
- If your child requires medication during the school day, please have the doctor complete the Physician Request for Medication Administration.
- Please sign the above form in the area labeled parent permission.
- All medications must be hand delivered to the nurse by an adult.
- Medication may not be transferred on the bus and children are not allowed to carry medication in school.
